PARENTAL CONSENT & RELEASE FORM

I the parent and/or legal guardian of  a student of Juja St. Peter’s School currently enrolled in Class/Grade do understand and agree to the following terms and conditions: 

1. Child(ren) Attendances
   • To have my child attend both in school and out of school academic and extra-curricular activities as provided by the school from time to time.
   • To have my child attend school activities, celebrations, ceremonies and other programs, as communicated by the school from time to time either individually, in the presence of other students and teachers and/or in my presence.

 

2. Health & Medical Information
   • In the case of my child requiring medical treatment or in the case of a medical emergency, I give consent and authorise the school’s management to undertake the following medical related procedures:
3. seek emergency medical care or treatment for my child when necessary;
4. perform first aid, including but not limited to applying plasters or dressings;
5. transport my child to a hospital for treatment where necessary;
6. procure medication on behalf of the child as instructed by qualified medical authorities;
7. where it is impracticable to communicate with me, to arrange for my child to receive such medical or surgical treatment as may be deemed necessary.
   • I undertake to pay any costs that may be incurred for the medical treatment, ambulance transportation and medications of my child.

 

3. The Use of Photographs, Digital Images, and Video and Audio Recordings
   • During the school year, we may take photos and videos of you and/or your child(ren) during learning, at play, at special events such as school trips, plays, meetings, festivals or celebrations to post internally in classrooms, corridors or notice boards, for educational and record keeping purposes. We additionally use the photographs and videos for our school newsletter, brochures, website and other official social media channels.
   • We undertake not to identify you and/or your child(ren) by name in the photographs or videos used for publicity purposes, unless for annual events such as commencement or publications such as the year book (if any). All photos and videos used for publicity will be available for the parents/ guardians to review upon request. In addition, parents/ guardians reserve the right to request that any photograph used for publicity, be taken down and/or removed, which request should be made in writing to Juja St. Peter’s School.
   • I understand:

• Why my consent is required.
• The reasons and conditions in which the school may wish to use photographs and videos of me and/or my child.
• This consent form for the use of photographs, digital images, and video/audio recordings is valid throughout the student’s stay at Juja St. Peter’s School and is not required to be resubmitted every academic year.
• The sharing or publication of the photos and/or videos will not be construed as invasion of mine or my child's privacy and the parent(s)/guardian(s) further absolve the school from liability arising from the use of my or my child's photo/video.
• Juja St. Peter’s School, as Data Controller under Data Protection laws, will allow the use of the images and videos in line with my consent.
• I can amend or withdraw my consent at any time by writing to the school principal.
• I understand that neither myself nor my child will not be identified by name when the photos or videos are used. I also understand that all photos will be available for review if I should request to do so.
  • I agree that the school will have no liability due to blurring, distortion, alteration, optical illusion or use in composite form that may occur in taking or usage of any picture or in the subsequent processing or publication of the picture.
  • I unconditionally consent to:

1. Give Juja St. Peter’s School permission to use photographs and videos of me or my child, taken either during class time, playground time, meetings, school events and functions, school trips and while within the school premises.
2. Photographs and videos of me and/or my child to be used for publicity & communication purposes on the school website, in brochures, newsletter or other means of publicity.
3. I release the school from all claims of any nature arising in any way from the use of you or your child’s photograph or video.

 

4. Data Collection, Usage and Protection
   • For the purposes of this clause, “Data” means name, nationality, gender, age, physical or postal address, identification details, email address, telephone/mobile number and any other personal information belonging to the parent or child that may be provided by the parent to the school on behalf of the child.
   • The School shall only use personal data for a specified and legitimate purpose and shall not process it in a manner that is contrary to those purposes. Any changes to the purpose for which the data is collected, processed and used shall be premised on a legitimate ground, and shall only be effected with the consent of the parent.
   • I unconditionally consent to:
5. the collection, use, processing and transfer by the school of the Data;
6. the use, processing, and transfer by the school of the Data to its advisors, authorized agents, the Ministry of Education and educational bodies/agencies assigned thereto; and
7. the transfer of the Data to third parties and the retention of the Data by the said third parties in connection with the child’s education.

ACCEPTANCE

I confirm that I am the parent or legal guardian of the child and that I have read and understood the contents of this parental consent form. I agree to comply with all the terms and conditions stated herein.
